Re: My plan did not match my own scope
Before writing this post I did something I recommend to everyone: I laid my Module 4 role analysis next to my Module 6 development plan and checked every goal against the scope I defended. In Module 4 I argued that as an adult-gerontology clinical nurse specialist in a hospital, most of my work would sit in the nursing practice and system spheres, not in managing individual patients' medical care (National Association of Clinical Nurse Specialists [NACNS], 2019). Two of my goals did not match that argument.
The first was a goal to complete an advanced pharmacology continuing education series so I could obtain prescriptive authority in my first year. When I reread my own analysis, I had said that my first position would not include prescribing and that doing so in my state would require a formal agreement with a physician and hospital privileges. The goal had come from a sense that real advanced practice means prescribing. I have replaced it with a goal to lead a medication-related practice change on my units, which fits the role I described. The national consensus model makes prescribing possible for the role but does not make it the point of the role (APRN Consensus Work Group & National Council of State Boards of Nursing APRN Advisory Committee, 2008).
The second was a goal to take a charge nurse leadership course so I could cover for the manager. That goal would pull me into staffing and personnel, which I had explicitly placed outside the role. I kept a leadership goal but changed it to a course on leading change without positional authority, which is what my role actually requires.
The biggest thing this course changed is that I now see scope as a commitment rather than a limit. Professional standards hold every registered nurse accountable for practicing within scope (American Nurses Association, 2021), and for me that now includes not drifting toward the tasks that feel most like authority.
