The clearest barrier to evidence-based practice I have seen is on my own unit. My unit is not unusual. In a national survey of more than 1,000 registered nurses, most said they valued evidence-based care, yet many reported barriers such as resistance from colleagues, nurse leaders and managers, and nurses in Magnet hospitals and those with master's degrees reported different experiences from other nurses (Melnyk et al., 2012). What struck me is that the barrier on my unit was not a lack of caring about evidence; it was that no one owned the question of whether our routine still made sense. Habits that everyone learned in orientation feel like policy even when they are not.
This course gave me a process I did not have before: turning a complaint into a question, searching and appraising the evidence, and presenting a recommendation in a way leaders can act on (Melnyk & Fineout-Overholt, 2023). My plan is small. I have asked our educator for fifteen minutes at the next practice council meeting to present a one-page summary comparing our policy with the current infusion standards (Gorski et al., 2021), and to propose that a council member review the policy with pharmacy. I am not asking the unit to change overnight, only to ask the question formally.
