The Part of My Job That Is Not in My Job Description
I manage a 26-bed inpatient oncology unit at a community hospital, and my job description is two pages of management: staffing, budgets, schedules, performance reviews, regulatory readiness. I do all of that, and it matters. But when I tracked my time for one week before writing this post, the work that actually changed anything for patients was almost entirely missing from those two pages.
Northouse (2021) defines leadership as a process in which one person influences a group of individuals to achieve a common goal, and separates it from management, which is about planning, organizing and controlling toward order and consistency. The distinction is useful because it shows where authority ends. I can manage my own staff through the schedule and the budget. I cannot manage the oncology pharmacist, the hospitalists, the physical therapists or the facilities engineers, and yet almost every problem on my unit runs through at least one of them.
Two examples from that week show the difference. Management was rebuilding the holiday schedule so that every shift had a chemotherapy-certified nurse. Leadership was persuading the pharmacy to change the time it delivers premixed infusions, so that our nurses stopped starting chemotherapy during shift change. I had no authority to ask for that. What I had was data on delays, a relationship with the pharmacy supervisor and an argument framed around the pharmacy's own goal of fewer wasted doses.
The evidence suggests that the relational side is not optional. Across 129 studies reviewed by Cummings and colleagues, relational styles kept turning up in the same places as nurses who liked their jobs and units where people wanted to stay, and a purely task-centered approach did not deliver the same results (Cummings et al., 2018). An earlier review linked relational and transformational leadership with outcomes that matter to patients, including lower mortality, fewer medication errors and fewer hospital-acquired infections (Wong et al., 2013).
My conclusion is that interprofessional leadership is mostly influence without authority, and the job description is silent on it because it cannot be assigned.
